Nice little story. I like the convenient plot device of the cousin having the different sides.
However, the story failed when the girl was told not to wear her glasses any more, of course that's probably because I have a megane fetish to go along with my onee-san and milf preferences.

I had to admit thought, I needed to flip back and forth between page three and four as I felt that a page was missing. …There wasn't. It actually gave me pause as I had to run though the senario in my head of how she would come to that conclusion; and how he didn't stop her at any point to explain his meaning before they were in the washroom.
I think this will remain as one of the most confusing and unforseen plot twists I've seen. The rest was average, but that moment makes this story memorable.

Now that I think about it, that's what was missing. A panel needed to be in this manga that seperated the ice-cream prediciment and the scene in the washroom that simply had that term in a rainbow to transition the scene. There would never have been a moment's confusion after that.
I'm not sure why, but the preview pic reminds me of that manga you shared, where one girl (and her sister) were forced to be maids (AKA sex slaves) to a wealthy asshole who bought their debt, and the guy freed her in the end because he realized she'd never love him.
But it's just that image, the girl's face looks a bit like the one from that manga. Anyway, nice share, I like the glasses too.
